ACodeRead® REWARDS CARD Account will be issued in the name provided first on the application. Only one account number is attached to each application. If a spouse wishes to have a separate account, a separate application must be submitted. Relatives and friends who wish to receive cards under your account number may do so, However, all Rewards credits earned by using Rewards cards with your account number will be credited directly to your personal account, accessible only by primary card holder. All Rewards Cards requested on this application will be delivered to the account holder(first name on application), together with your personal account number and access code. Account holder will be responsible for distributing these extra cards to friends and relatives.
